Contracting Process

1	Identify if PEHP has an open panel for your specialty and area.
2	Request an application from PEHP.
3	PEHP credentials provider.
4	Contract is sent to credentialed provider.
5	Provider reads and signs provider agreement.

The effective date of participation is based upon when PEHP receives the signed agreement. PEHP will make the effective date the 1st of the month if received by 15th of the month, otherwise it will be the first of the following month.
