Care and Disease Management

Disease management services are provided by MEDCO/Optimal Health, a nationally recognized leader in disease management support.

Patients who are identified as having diabetes, COPD, heart failure or coronary artery disease, by their Pharmacy, or medical claims are engaged over the phone and by mail to help them better understand and manage their disease.
