Application

PEHP is currently accepting applications from medical providers in the following specialties in the Wasatch front:  psychiatry, endocrinology and rheumatology. For rural areas outside the Wasatch front, other specialties may be needed. We also will accept applications if you are joining an existing contracted practice or taking over a practice of a contracted provider. If you meet the above criteria, please contact the Provider Relations Specialist in your area including your name, tax identification number, specialty, location and phone number. We also accept UHINSpeedi applications.
