Pre-authorization

Your Benefits Summary will tell you which services require pre-authorization. To get pre-authorization, call PEHP at 801-366-7755 or 800-753-7754. It’s important not to forget this step. Failure to pre-authorize may result in a reduction in benefits, or your claim may be denied entirely. Access your Benefits Summary at myPEHP.
